We want to support you and your loved ones to live independently and feel part of the community and welcome any referrals for our services. If you or your loved one is in need of our service, then please contact us today.

We expect referrals from social services, other care organisations, hospitals and individuals from the public. Our thorough referral system means, working with individuals and organisations to plan bespoke services that suit their needs. Our Care Manager is available to give you support so you can make informed decisions and choices about using the services that best suits your needs.

ASSESSMENTS

Before accessing our services, our Care Managers will offer you unbiased expert psycho-social assessment of your care and support needs free of charge. Our manager will meet with you at a place of your choice, to discuss your individual requirements, preferences and expected outcomes. After this assessment we will be able to suggest appropriate services that best meet your needs.

During the assessment:

  • We will seek your views and wishes of the type of services you require.
  • You may have a friend, relative or someone else to help you or speak for you.
  • We will arrange for an interpreter if you do not speak English or if you use sign language.
  • We will consider your relative’s views and if you are a child, your parents’ views.
  • We will give you information about the services that are available and any changes involved.

After your assessment, we will tell you:

  • Which services are available to you, who will provide them, where and when they will be provided as per agreement between the two parties.
  • The name of the person who is your caregiver and the Care Manager who is responsible for managing your care.
